You must enter your email address for the Work Order to be processed properly.  You will receive a confirmation email that your Work Order was received once you click on the "Submit" button.

Make sure you enter your full and complete email address, otherwise you will get a blank screen when you click on the submit button and your work order will not be sent in.

Make sure all information in all fields is accurate and complete before you click on the "Submit" button

If you leave any fields blank or do not enter your email address, your Maintenance Work Order may not be processed properly.  If you click on the "Submit" button and you are taken to a blank white screen, some fields of information were not entered properly or you did not enter your email address.  You would need to return to the Maintenance Online Work Order form and fill it in completely and then click on the "Submit" button again.
