REGISTRATION INFORMATION
All forms in the left column below need to be completed and returned to school the week of August 2nd - 6th. Print, completely fill out and return the forms to S.M.S. We are unable to accept forms electronically. The office will re-open starting August 2 for regular business hours, 7a.m. - 3:30 p.m. For your convenience we will stay open until 6:30 p.m. on Wednesday, August 4. After dropping off the forms you may take time to walk around, find classrooms, and try out the locker.
