Cost
is $25.00 per student athlete

Student
athletes
must check in upon arrival and will be handled on a first come basis.
IHSAA Pre-Participation Physical Evaluation Forms will be
available at time of the physical.
Physical forms can also be picked up prior to the examination in
the front offices of SMS and SHS.

The
physical form must be signed by a parent/guardian for all
student athletes under 18 years old.
Come prepared to provide shot history.

All
students interested in participating in athletics at the middle school or
high school, must have a physical on file before being allowed to
participate in official practices or try-outs. Physicals are not
necessary for participation in camps, clinics or open facility sessions
during the summer.
