2009 IHSAA Sports Physical Examinations

 Will be Offered for Shelbyville Central Schools Incoming Student Athletes in Grades 6-12

** Cost is $25.00 per student athlete **

  • Student athletes must check in upon arrival and will be handled on a first come basis.

  • Students are asked to come to the examination prepared with an IHSAA Pre-Participation Physical Evaluation Form with page one completed.

  • Link to printable copy of IHSAA Physical Form: PhysicalForm09.pdf 

  • Physical Forms may also be picked up in the front offices of SMS and SHS as well as the SHS Athletic Department. 

  • The physical form must be signed by a parent/guardian for all student athletes.  Come prepared to provide immunization history.  

  • All SMS & SHS students interested in participating in athletics for the 2009-10 sports season at the middle school or high school, must have a signed physical on file in the athletic department before being allowed to participate in official practices and/or try-outs.  Physicals are not necessary for participation in camps, clinics or open facility sessions during the summer.

2009 Golden Bear Youth Sport Camps

Below is a listing of the Youth Sport Camps that are being offered at Shelbyville High School with their respective head coaches. You may pick up registration and enrollment forms for each camp at the following locations:

Coulston Elementary, Loper Elementary, Hendricks Elementary, Shelbyville Middle School, Shelbyville High School, Shelbyville Boys Club, and Girls Inc.

You may register the 1st day of camp. If you need further information, please call the SHS Athletic Office at 398-9731, ext. 2117 or 2118.
