Immunization Requirements for the 2010-2011 School Year

Message from Nurse Herbert, SCS Health Services Director.

All preschool and kindergarten students will be required to have 2 doses of varicella (chickenpox) vaccine or physician written documentation of history of chickenpox disease, including month and year of disease.

All kindergarten students will be required to have one of the required doses of polio vaccine, given on or after the fourth birthday.

All 6th-12th grade students will be required to have 1 dose of tetanus-diphtheria-acellular pertussis vaccine (Tdap), 1 dose of meningococcal conjugate vaccine (MCV4), and 2 doses of varicella (chickenpox) vaccine or a signed written statement from parent/guardian indicating history of chickenpox disease including month and year of disease.

For questions, please contact your child’s physician or the Shelby County Health Department at 392-6470.

Once your child’s immunizations are updated, please submit a copy to your child’s school or have immunization record available at registration.

Your child must have had the immunizations or an appointment scheduled prior to the first day of school.

SMS & SHS Registration (Grades 6-12)

Beginning July 26th, registration forms will be available online on your school website. New student enrollment will be by appointment beginning August 2nd. Contact your school's office, 392-2551 (SMS) or 398-9731 (SHS) to set an appointment.

Wednesday, August 4 - Freshman Orientation begins at 10:00 AM in Breck Auditorium
~ Teachers will greet incoming freshman in the front lobby ~

**SCS will follow a Regular School Day schedule on August 11. The Wednesday Early Release schedule will begin the following week on August 18 (SMS & SHS at 2:00 PM & Elementary at 2:40 PM).
